

When a loved one leaves hospital after a stay, to return home, it can be a daunting time. Our Hospital Liaison Worker, who works in partnership with Borders General Hospital, is there to ensure this transition is as smooth as possible. She will visit the patient , if appropriate, and talk with you, the carer, in order to fully understand your personal circumstances. She will then work with you intensively for 6 weeks to make sure all the support and services are in place. Then you will be ready to welcome your loved one home and continue your caring role in a healthy, happy way.
